Job summary

Central Highlands Rural Health invites applications for a District Nurse (Registered Nurse) in the Macedon Ranges Community Nursing program. The role focuses on providing comprehensive nursing care in clients' homes and community settings within Sunbury and surrounding areas.

As a District Nurse, you will work autonomously with support from an experienced team, managing a diverse caseload across CHSP, HACC-PYP, DVA, and related programs.

Qualifications

• Registered Nurse with current Australian practicing certificate.
• Experience delivering home-based nursing care preferred.
• Able to work autonomously within a supportive team.

Responsibilities

• Provide comprehensive clinical nursing care in clients’ homes and community settings.
• Manage a diverse caseload across CHSP, HACC-PYP, DVA, TCP, and Support at Home.
• Deliver person-centred care to promote health, wellbeing and independence.
• Collaborate with the Macedon Ranges Community Nursing team, including HITH and Palliative Care options.

We are seeking a motivated and compassionate Registered Nurses to join our growing District Nursing Service. This is an opportunity to provide high-quality nursing care in the beautiful Macedon Ranges, supporting our community to remain healthy, anjd living independently and safely in their own homes.

As a District Nurse, you will work autonomously while being supported by an experienced team. You will manage a diverse caseload across a range of community programs, delivering person-centred care to clients.
