Job summary

The District Nurse position within Macedon Ranges Community Nursing delivers comprehensive clinical nursing care in clients' homes and community settings across CHSP, HACC-PYP, DVA, TCP and Support at Home programs to promote health, wellbeing and independence.

You will work autonomously while being supported by an experienced team, managing a diverse caseload and delivering person-centred care to clients. Opportunities include brokered HITH and Community Palliative Care within the team.
