Central Highlands Rural Health is seeking motivated Registered Nurses to join our District Nursing Service in the Macedon Ranges. This role supports our community to stay healthy and live independently at home, delivering high-quality, person-centred care.

You will work autonomously within an experienced team across CHSP, HACC-PYP, DVA, TCP and Support at Home programs, promoting wellbeing and independence while exploring brokered HITH and Community Palliative Care opportunities.

Responsibilities

• Provide district nursing care in clients' homes and community settings.
• Manage diverse caseload across CHSP, HACC-PYP, DVA, TCP and Support at Home programs with person-centred care.
• Work autonomously within an experienced team.
• Explore brokered HITH and Community Palliative Care opportunities within the team.
