Department of Health Tasmania is seeking a permanent full-time Clinical Nurse Consultant – Discharge Planning to provide expert clinical leadership in safe, timely discharge from the North West Regional Hospital. You will work with Patient Flow and After hours Managers, aged care and community teams to resolve complex discharge needs and strengthen pathways between services.
