The Department of Health Tasmania invites applications for a senior allied health professional to join the Complex Care team at Launceston General Hospital, working within rehabilitation services to support the right place, right care journey for patients with complex needs.
You will provide expert clinical advice on care management, complex discharge planning, and hospital avoidance, collaborating with nursing, medical and external providers to achieve safe, patient-centred outcomes and timely
Qualificações
- Satisfactory completion of an approved allied health professional qualification/program and registration with the relevant National Board.
- Postgraduate qualifications relevant to the position.
- Must be registered or eligible for registration with the relevant professional board.
Responsabilidades
- Lead complex patient journeys and high-risk discharge planning across multiple services and stakeholders.
- Provide clinical leadership beyond case management, including coaching and service-improvement initiatives.
- Analyse outcomes and data to drive system-wide solutions for highly complex health, psychosocial and NDIS-related needs.
