The Department of Health Tasmania seeks an experienced allied health professional to join the Complex Care team based predominantly at Launceston General Hospital. You will work with nursing, medical and allied health colleagues and external providers to support the right place, right care patient journey and quality outcomes.

Qualifications

Satisfactory completion of an approved allied health professional tertiary qualification/program of study and registered with the relevant National Board
Desirable: post graduate qualifications relevant to the position

Responsibilities

Provide expert clinical advice on care management and systems pathways
Lead complex patient journeys and coordinate cross-service discharge planning
Drive service improvement initiatives and contribute to system-wide solutions for patients with highly complex health, psychosocial, behavioural, and NDIS-related needs
