Job summary

Discover Nursing Australia in Rosebud, VIC is seeking a Disability Support Worker to provide in-home support and community access for an ongoing 1:1 participant. The role includes meal prep, household tasks, and transport to activities.

We value reliability, empathy, and professionalism. Training and coordination support are provided to ensure quality, person-centred care and adherence to the participant's plan.

Qualifications

• Certificate III in Individual Support or equivalent qualification.
• NDIS Worker Screening Check completed or willing to obtain.
• National Police Check current or in-process.
• Current First Aid and CPR certifications.
• NDIS Worker Orientation Module completed.
• ABN or willingness to obtain one to contract.

Responsibilities

• Assist with meal preparation according to participant's preferences.
• Complete ironing and other household tasks.
• Support shopping and community outings.
• Provide transport for activities and family visits.
• Offer companionship and respectful social engagement.
• Encourage independence and respect participant choice.
• Follow the participant's support plan and routines.
• Maintain progress notes and support documentation.
