Job summary

Discover Nursing Australia is seeking a reliable Disability Support Worker to provide in-home support and community access for a participant in Rosebud, VIC. The role combines household tasks with transportation and social outings to promote independence and well-being.

You will work 1:1 with the participant, following a support plan, documenting progress, and coordinating with our nurse and team to ensure consistent, respectful care.

Qualifications

• Certificate III in Individual Support or equivalent.
• NDIS Worker Screening Check is current.
• National Police Check is current.
• Current First Aid Certificate and CPR.
• NDIS Worker Orientation Module completed.

Responsibilities

• Assist with meal preparation and household tasks.
• Complete ironing and other agreed tasks.
• Support shopping and community outings.
• Provide transport for activities and family visits.
• Offer companionship and respectful social engagement.
• Encourage independence and participant choice.
• Follow the support plan and routines.
• Maintain progress notes and documentation.
