Job summary

DMS Care is seeking a compassionate Disability Support Worker to provide person-centred support to participants in Belfield or Glebe. You will help with daily living, accompany them to appointments and community activities, and support their independence in a respectful, confidential manner.

We value reliable individuals with good communication, documentation and relationship-building skills. A Certificate III or IV in relevant fields is preferred, plus current checks and a valid driver’s

• Experience in disability, aged care, community services or similar support role.
• Current unrestricted driver’s licence and access to a reliable vehicle.
• NDIS Worker Screening Check and Working With Children Check where required.
• National Police Check and up-to-date First Aid/CPR certificates.

Responsibilities

• Provide person-centred support based on each participant's needs and goals.
• Assist with daily living activities, personal care and household tasks.
• Support participants with appointments, shopping, social activities and community access.
• Encourage independence, choice and meaningful community participation.
• Follow participant care plans and medication procedures.
• Maintain accurate progress notes and required documentation.
• Communicate professionally with participants, families and the DMS team.
• Identify and report changes in health or support needs.
• Maintain participant safety, privacy, and confidentiality.

Disability Support Worker - Belfield or Glebe

Make a real difference every day by providing person-centred support that promotes independence, dignity and community connection.

DMS is seeking compassionate, reliable and experienced female Disability Support Worker to support participants in Belfield or Glebe. This is more than simply completing daily tasks- you'll build trusted relationships and help participants achieve their goals, develop independence and remain connected to their communities.

Our participants have individual needs, preferences and routines. You'll be expected to provide respectful, consistent support while following their care plans, behaviour support strategies and personal choices.

You'll Be Responsible For

• Providing person-centred support based on each participant's individual needs and goals
• Assisting with daily living activities, personal care and household tasks when required
• Supporting participants with appointments, shopping, social activities and community access
• Encouraging independence, choice and meaningful community participation
• Following participant care plans, behaviour support plans and medication procedures
• Maintaining accurate progress notes, incident reports and other required documentation
• Communicating professionally with participants, families and the DMS team
• Identifying and reporting changes in a participant's health, wellbeing or support needs
• Maintaining participant safety, privacy, dignity and confidentiality at all times

What You'll Need

• Previous experience in disability, aged care, community services or a similar support role
• Certificate III or IV in Individual Support, Disability or Community Services is preferred
• A compassionate, patient and professional approach
• Strong communication, documentation and relationship-building skills
• Ability to work independently and follow established support plans and procedures
• Current unrestricted driver's licence and access to a reliable, insured vehicle
• Current NDIS Worker Screening Check
• Working With Children Check, where required
• National Police Check
• Current First Aid and CPR certificates
