Job summary

Lutheran Disability Services supports people with disability and their families with accommodation support (including Supported Independent Living) and community participation. We help clients have fun, connect to their community, learn new skills and increase independence.

The Support Worker role involves compassionate, respectful support to clients, developing personalised care plans, and collaborating with families and professionals to promote independence and inclusion.

Qualifications

• Strong interpersonal and communication abilities.
• Ability to empathise and provide emotional support to individuals with disabilities.
• Problem-solving skills to address client challenges.
• Capability to assist with daily living activities including personal care and mobility.

Responsibilities

• Assist clients with daily living activities such as personal hygiene, meal preparation, and medication management.
• Support clients in accessing community resources and services, including educational programs and recreational activities.
• Develop individualised care plans in collaboration with clients, families, and other professionals to meet specific needs.
• Provide emotional support and encouragement to help clients achieve their personal goals.
• Document client progress and report any changes in behaviour or health status to the appropriate team members.
• Ensure a safe environment by following health and safety protocols, reporting hazards, and maintaining cleanliness.
• Engage in regular training sessions to stay updated on best practices in disability support services.
