Job summary

Chosen Family is seeking an experienced Support Worker to assist a 30-year-old male participant with Duchenne Muscular Dystrophy in a home setting. You will provide person-centred care, assist with daily living tasks, and help maintain independence while ensuring safety and dignity.

Qualifications

• NDIS Worker Screening Check required
• Current First Aid Certificate required
• Relevant disability support or personal care experience essential
• Experience with significant physical disability/complex care highly regarded
• Availability for advertised shifts is important

Responsibilities

• Provide person-centred disability support in the participant's home
• Assist with personal care and activities of daily living
• Support safe wheelchair transfers according to established procedures
• Assist with mobility and positioning as required
• Maintain dignity, comfort and independence for the participant
• Assist with domestic tasks and household routines
• Support meal preparation and other daily living activities as needed
• Follow manual-handling and workplace safety procedures
• Help the participant engage in usual routines and activities
• Communicate professionally with the participant, family and support team
• Maintain shift notes and required documentation
• Report concerns or changes affecting safety or support
