MS Queensland is seeking experienced and compassionate Disability Support Workers to provide in-home and community-based care for individuals with high and complex support needs in Toowoomba and surrounding areas.

You will work across a 24/7 rotating roster, delivering person-centred care in clients’ homes and local apartments, following care plans and NDIA guidelines while collaborating with the care team.

Qualificações

• Cert III in Disability or equivalent; First Aid/ CPR up-to-date
• Experience with high/complex needs and neurological conditions is preferred
• NDIS understanding or willingness to learn and commitment to dignity and choice

Responsabilidades

• Deliver high-quality personal care and daily living support (mobility, meals, domestic tasks)
• Support community access and meaningful participation to build independence
• Provide medication support and assist with medical appointments as required
• Deliver complex supports in line with care plans (catheter/bowel care, hoisting/transfers, PEG feeding, suctioning and more)
• Monitor wellbeing, document accurately, and elevate health/behaviour changes promptly
• Maintain clear records and follow individual care plans, policies and safe work practices
• Work collaboratively with the team to deliver consistent, safe, person-centred support every shift
