Job summary

Discover Nursing Australia is seeking a reliable Disability Support Worker to provide 1:1 community access support for a young gentleman in Queensland. The role is offered as an independent contractor, with ABN and nurse-led coordination.

You will accompany appointments, go shopping, visit cafés, parks, and participate in activities, while promoting independence and social participation per the participant's goals.

Qualifications

• Certificate III in Individual Support or equivalent qualification is required.
• Commitment to person-centred practice and independent support.
• Current NDIS Worker Screening Check is required.
• Current National Police Check and First Aid/ CPR.
• NDIS Worker Orientation Module completion.

Responsibilities

• Support attendance at appointments.
• Assist with shopping and errands.
• Accompany the participant to cafés, parks and local attractions.
• Support participation in recreational activities and personal interests.
• Provide transport for community activities.
• Offer companionship and encourage social engagement.
• Follow the participant’s support plan and maintain progress notes.
• Communicate observations or concerns to the coordination team.
• Ensure safety and wellbeing during outings.

About the Opportunity

We are seeking a reliable, personable and professional Disability Support Worker to provide 1:1 community access support to a young gentleman.

The role involves supporting the participant to attend appointments, visit cafés, go shopping and participate in recreational activities of his choice. The successful applicant will provide companionship while encouraging independence, confidence and social participation.

Building rapport and providing consistent, person-centred support are central to this role.

Key Responsibilities

• Support attendance at appointments.
• Assist with shopping and everyday errands.
• Accompany the participant to cafés, parks, local attractions and other agreed destinations.
• Support participation in recreational activities and personal interests.
• Provide transport for agreed community activities.
• Offer companionship and support positive social engagement.
• Encourage participant choice, independence and confidence.
• Work towards the participant’s identified NDIS goals.
• Follow the participant’s support plan and maintain accurate progress notes.
• Communicate relevant observations or concerns to the coordination team.

Essential Requirements

Applicants must hold, or obtain before commencing where applicable:

• An active Australian Business Number (ABN).
• Australian working rights.
• Certificate III in Individual Support or an equivalent relevant qualification.
• Current NDIS Worker Screening Check.
• Current National Police Check.
• Current First Aid and CPR certification.
• Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
• Current driver’s licence.
• A reliable, comprehensively insured vehicle suitable for transporting the participant.
