Job summary

Discover Nursing Australia is hiring a Disability Support Worker to deliver 1:1 community access support for a young gentleman. You will accompany him to appointments, cafés, shops and recreational activities, providing companionship and encouragement for independence.

The role is offered to contracted workers with ABN, supported by our nurse-led team. You will help achieve NDIS goals and maintain progress notes in line with the support plan.

Qualifications

• Must hold an active ABN and Australian working rights.
• Relevant disability support qualifications and checks are required.
• Current First Aid and CPR certification must be maintained.
• NDIS Worker Orientation and background checks are mandatory.

Responsibilities

• Provide 1:1 community access support to the participant.
• Assist with appointments, shopping and errands.
• accompany the participant to cafés, parks and activities.
• Support participation in recreational activities and personal interests.
• Provide transport for community activities as needed.
• Offer companionship and encourage independence and social participation.
• Follow the participant’s plan and document progress notes.

About the Opportunity

We are seeking a reliable, personable and professional Disability Support Worker to provide 1:1 community access support to a young gentleman.

The role involves supporting the participant to attend appointments, visit cafés, go shopping and participate in recreational activities of his choice. The successful applicant will provide companionship while encouraging independence, confidence and social participation.

Building rapport and providing consistent, person-centred support are central to this role.

Key Responsibilities:

• Support attendance at appointments.
• Assist with shopping and everyday errands.
• Accompany the participant to cafés, parks, local attractions and other agreed destinations.
• Support participation in recreational activities and personal interests.
• Provide transport for agreed community activities.
• Offer companionship and support positive social engagement.
• Encourage participant choice, independence and confidence.
• Work towards the participant’s identified NDIS goals.
• Follow the participant’s support plan and maintain accurate progress notes.
• Communicate relevant observations or concerns to the coordination team.

Essential Requirements:

Applicants must hold, or obtain before commencing where applicable:

• An active Australian Business Number (ABN).
• Australian working rights.
• Certificate III in Individual Support or an equivalent relevant qualification.
• Current NDIS Worker Screening Check.
• Current National Police Check.
• Current First Aid and CPR certification.
• Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
• Current driver’s licence.
• A reliable, comprehensively insured vehicle suitable for transporting the participant.
