Job summary

Discover Nursing Australia is seeking a reliable Disability Support Worker to provide 1:1 community access support for a young gentleman. You will accompany him to appointments, cafés, parks and activities, providing companionship while promoting independence and social participation.

This contracted role is ABN-based and supported by our nurse-led coordination team. Applicants must hold an active ABN, Certificate III in Individual Support or equivalent, and current checks (NDIS Worker

Qualifications

• Active ABN is required.
• Certificate III in Individual Support or equivalent.
• Current NDIS Worker Screening Check.
• Current National Police Check.
• Current First Aid and CPR certification.
• Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
• A comprehensively insured vehicle suitable for transporting the participant.

Responsibilities

• Support attendance at appointments.
• Assist with shopping and everyday errands.
• Accompany to cafés, parks, local attractions and other agreed destinations.
• Support participation in recreational activities and personal interests.
• Provide transport for agreed community activities.
• Offer companionship and support positive social engagement.
• Encourage participant choice, independence and confidence.
• Work towards the participant’s identified NDIS goals.
• Follow the participant’s support plan and maintain accurate progress notes.
• Communicate relevant observations or concerns to the coordination team.

About the Opportunity

We are seeking a reliable, personable and professional Disability Support Worker to provide 1:1 community access support to a young gentleman.

The role involves supporting the participant to attend appointments, visit cafés, go shopping and participate in recreational activities of his choice. The successful applicant will provide companionship while encouraging independence, confidence and social participation.

Building rapport and providing consistent, person-centred support are central to this role.
