Job summary

Harmony Support Australia is seeking experienced Support Workers to provide person-centred supports in the Brisbane Southside area. You will deliver trauma-informed, person-centred care that promotes independence and community participation.

You will work collaboratively with families and the broader team, implementing BSP/PBSP strategies and maintaining accurate records. The role emphasizes safety, respect, and high-quality support across complex needs and settings.

Harmony Support Australia is seeking experienced, compassionate, and resilient Support Workers to join our team to provide person-centred supports in Brisbane Southside area.

What you'll be doing

As a Support Worker, you will:

• Deliver person-centred support in accordance with the participant's NDIS goals and preferences.
• Implement Positive Behaviour Support strategies built in Behaviour Support Plans (BSP/PBSP).
• Build a trusting and therapeutic relationship with the participant.
• Support community access, appointments, daily living skills, and social participation.
• Maintain accurate case notes and incident reporting.
• Work collaboratively with families, behaviour support practitioners, and the broader multidisciplinary team.
• Promote a safe, respectful, and positive environment at all times.

What we're looking for

• Ability to manage complex mental health situations
• High intensity supports, Personal care, Hoist /Sera steady experience
• Current NDIS Worker Screening Check
