Momentum Community Connections – Melton VIC

The Disability Support Worker is responsible for delivering support services aimed at enhancing the living, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

Key responsibilities

Provide assistance with households as outlined in the participants plan and encourage them to participate in those tasks actively

Provide assistance with community participation for our participants to reach their goals and objectives

Provide support that will enable participants to actively engage in the community or in any daily personal activities of their own choice and interest

Create a safe, enjoyable and creative environment at all times

Maintain an environment which promotes participants decision making skills and contribute to their personal growth

Show empathy and respect for the dignity, rights, and care of all the participants

Actively always engage with participants

About you

The successful candidate must be able to support and meet the needs of NDIS participants who identify as LGBTIQA+ and their families.

Must have or be willing to obtain;

NDIS Screening Check, WWCC, First Aid and CPR

NDIS Orientation Modules

Driver’s licence and insured vehicle

The primary participants require consistency and the successful candidate for this role will be able to commit to regular ongoing shifts with flexibility to cover emergency shifts as required,

Available shifts typically include;

Sleepovers 7pm to 10am (Monday to Sunday)

Morning 10am to 3pm / Afternoon 12pm to 7pm
