Job summary

Nexa Care is seeking reliable, professional Disability Support Workers to join our growing team in Victoria, Australia. You will provide direct support to participants with complex needs in SIL homes and community settings, maintaining professionalism and adherence to established support frameworks.

Applicants with at least 6 months in disability or aged care, knowledge of NDIS Practice Standards, and strong communication and documentation skills are preferred.

Responsibilities

• Deliver person-centred supports aligned with individual plans and BSPs
• Assist with Activities of Daily Living, including personal care
• Support community access and social engagement
• Implement positive behaviour support strategies per authorised practices
• Document supports, incidents, and observations clearly in FlowLogic
• Participate in structured handovers and team communications
• Maintain a safe environment and escalate risks as needed

About the Role

You will provide direct support to participants with complex needs, including physical, cognitive, and behavioural support requirements, within SIL homes and community environments.

This role requires a high level of professionalism, consistency, and adherence to established support frameworks.

Key Responsibilities

Deliver person-centred supports aligned with individual support plans and Behaviour Support Plans (BSPs)

Provide assistance with Activities of Daily Living (ADLs), including personal care

Support participants with community access and social engagement

Implement positive behaviour support strategies in line with authorised practices

Accurately document all supports, incidents, and observations in FlowLogic

Participate in structured handovers and team communication processes

Maintain a safe environment and escalation risks, incidents, or changes in participant presentation

What We're Looking For
