Job summary

LEAP Support Services Tasmania is seeking a hands-on Support Leader to drive day-to-day care and on-site team leadership across our North-West Coast sites. You will deliver hands-on, participant-focused support while coaching Support Workers to keep homes and routines running smoothly.

This role requires a Certificate IV in Disability (or equivalent), strong communication, and the ability to manage competing priorities.

Qualifications

• Certificate IV in Disability or equivalent (or working towards).
• Experience in disability support with complex needs.
• Strong communication skills and person-centred approach.
• Current driver’s license and ability to obtain required checks (NDIS Worker Screening, Working with Children, vulnerable people registration).
• Ability to manage competing priorities under pressure.

Responsibilities

• Deliver hands-on, person-centred support to participants across sites.
• Provide leadership and coaching to Support Workers across sites.
• Lead by example, embedding LEAP values and person-centred practice.
• Ensure care plans, behaviour support plans, and documentation are current and followed.
• Maintain a visible, hands-on presence across sites and shifts.
• Coordinate participant appointments and keep records accurate and up to date.
• Own accurate, timely reporting, including medication management and end-of-month documentation.
• Identify and elevate risks, incidents, and wellbeing concerns as they arise.
• Seek guidance from Supervisor for timely updates across sites.
• Build genuine, trusted relationships with participants and families.
• Be a reliable contributor to the after-hours on-call roster.

Founded in 2014 and grounded in a person-centred approach, LEAP has grown into one of the leading NDIS providers on Tasmania's North-West Coast - offering Supported Independent Living, Short and Medium-Term Accommodation, 1:1 community support, and Coordination of Supports.

Which brings us to you. We're after an experienced support worker who leads by doing, who's just as comfortable delivering exceptional care to participants while also leading a team of Support Workers who are responsible for keeping the lives and homes of our participants running smoothly. This isn't a role where you sit back and direct from a distance; it's hands-on, on-the-ground leadership where your presence and consistency genuinely shape the experience of every participant and Support Worker on your sites.

About the role

As Support Leader, you'll provide day-to-day support to participants along with on-site leadership to Support Workers, ensuring services are delivered safely, consistently, and in line with participant needs and plans. You'll be the visible, steady presence across your sites - supporting your team, championing quality outcomes for participants.This role is based between Wynyard to Latrobe.

Day to day, you will:

• Deliver hands-on, person-centred support to participants across your sites
• Provide leadership, and coaching to Support Workers across your allocated sites
• Lead by example, embedding LEAP's values and person-centred practice in everything your team does
• Ensure care plans, behaviour support plans, and documentation are current, understood, and followed
• Maintain a visible, hands-on presence across your sites and varying shifts
• Coordinate participant appointments and keep relevant documentation accurate and up to date, communicating changes to Support Workers
• Own accurate, timely reporting, including medication management and end-of-month documentation
• Identify and elevate risks, incidents, and participant wellbeing concerns as they arise
• Seek guidance & input from Supervisor to ensure timely updates are maintained across all sites
• Build genuine, trusted relationships with participants and their families
• Be a reliable contributor to the organisation's after-hours on-call roster

About you

You're a natural leader who thrives on the frontline. You'll bring:

• Certificate IV in Disability, Community Services, or equivalent (or working towards)
• Demonstrated experience in disability support with complex needs
• Strong communication skills and a genuine, person-centred approach
• The ability to manage competing priorities, while remaining calm under pressure
• A current driver's license, and the ability to obtain (or hold) a NDIS Worker Screening Check, Working with Children Check, and vulnerable people registration
