Pathway Talent is seeking a casual Concierge Disability Support Worker / Enrolled Nurse to join our premium, participant‑centred team in Sydney. You will build trusted relationships with a small group of participants, support daily living activities, and work closely with families and health professionals.

Ideal candidates possess Certificate III or IV in Individual Support or Disability or Diploma of Nursing with current AHPRA registration, plus NDIS Worker Screening, WWCC, and First Aid/CPR.

• Certificate III or IV in eligible field required.
• Diploma of Nursing with current AHPRA registration for Enrolled Nurse.
• Current NDIS Worker Screening Check, Working With Children Check and First Aid/CPR.

In this role, you will:

• Build and maintain professional, participant-focused relationships with a small group of participants.
• Understand each participant’s preferences, routines, goals, and aspirations.
• Coordinate and facilitate supports that enhance independence and quality of life.
• Act as a proactive problem solver, identifying opportunities to improve participant outcomes.
• Provide a responsive and professional service that focuses on participant experience.
• Collaborate closely with families, guardians, support coordinators, allied health professionals, and internal teams.
• Support participants with personal care, daily routines, and activities of daily living while maintaining their dignity and choice.
• Deliver support that is flexible, innovative, and tailored to individual needs rather than a "one size fits all" approach.

Essential Requirements

• Certificate III or IV in Individual Support, Disability, Community Services, or equivalent (Support Workers).
• Diploma of Nursing and current AHPRA registration (Enrolled Nurses).
• Current NDIS Worker Screening Check.
• Current Working With Children Check.
• Current First Aid and CPR Certificate.
• National Police Check.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Ability to lift 10kg unassisted.
• Experience in Concierge model a bonus but not essential.
