Job summary

Healthscope Independence Services seeks a passionate Disability Support Worker to provide high-quality care in Supported Independent Living settings under the NDIS. You will assist with daily living activities, personal care, and community engagement, contributing to clients' independence and well-being.

Qualifications

• Certificate IV in Disability, Community Services, or related qualification.
• Minimum of 1 year of experience in disability or healthcare services is required.
• Strong communication, written, interpersonal, and organisational skills.
• Flexibility to work across a range of shift types.

Responsibilities

• Provide personal care support in line with individual support plans.
• Assist participants with daily living activities in meal preparation and feeding.
• Assist with household tasks such as cleaning, laundry, etc.
• Accompanying and supporting clients in community-based activities, such as social outings, appointments, and recreational pursuits.
• Ensure all required documentation (including daily notes, communication and progress reports) is thorough and up to date.
• Work within a team environment.
