Job summary

Vital Support Services is seeking a dedicated Disability Support Worker to provide person-centred care for participants with complex needs in the Brisbane area. You will work with participants, families and the care team to promote independence, safety and meaningful participation in daily life.

The role requires compassionate care, ability to follow individual plans, and collaboration with multidisciplinary teams. Current First Aid/CPR and NDIS screening are required.

Qualifications

• Experience supporting people with complex physical disabilities and high-intensity support needs.
• Ability to follow individual care plans and health care procedures.
• Strong written English and clear, timely participant documentation.
• Current First Aid and CPR certification and NDIS Worker Screening clearance.

Responsibilities

• Assist with activities of daily living and personal care while maintaining dignity and privacy.
• Provide complex personal care per participant care plans and training.
• Support medication management as per care plan and organisational requirements.
• Assist with mealtime support and health-related needs.
• Support household tasks and community access for meaningful engagement.
• Document changes in health or wellbeing and report accordingly.
• Collaborate with families, care coordinators, nurses and allied health professionals.
