Job summary

Caring Your Home Pty Ltd is seeking Disability Support Workers to provide direct support to NDIS participants in their homes and in the community. You will assist with daily living activities, personal care, meal preparation, and community access while following individual support plans.

The role emphasizes dignity, safety, and wellbeing; Mandarin speaking ability is highly regarded. Certificates and background checks are required, with flexible shifts and ongoing training offered for

Qualifications

• Certificate III or IV in Disability Support, Individual Support, Aged Care, Community Services or related field preferred.
• Previous experience in disability support, aged care, personal care, community care or healthcare support desirable.
• Understanding of NDIS participant rights, choice and control, privacy, dignity and person-centered support.
• Good communication and interpersonal skills.
• Ability to follow care plans, instructions and workplace procedures.
• Ability to work independently and report issues appropriately.

Responsibilities

• Assist participants with daily living activities including showering, dressing, grooming, toileting, mobility, transfers and other personal care needs.
• Support participants with meal preparation, feeding assistance, hydration and medication prompting where required and within your scope of work.
• Assist with domestic tasks including cleaning, laundry, changing bed linen, shopping and household support.
• Support participants to access the community, attend appointments, participate in social activities and engage in recreational programs.
• Assist participants to develop daily living skills such as personal hygiene, cooking, budgeting, communication and community participation.
• Encourage participants to work towards personal goals including independence, social participation, education or employment-related goals.
• Follow individual support plans, care plans, behaviour support strategies and company policies/procedures.
• Maintain accurate service notes, progress notes, incident reports and other required documentation.
• Report concerns, incidents, changes in participant condition, risks or safeguarding concerns to the office or management team.
• Work respectfully with participants, families, carers, support coordinators, healthcare professionals and other service providers.
• Attend training, supervision and team meetings as required.
