Job summary

Equality Disability & Health Care Services is seeking Disability Support Workers to assist participants in Community Access and Supported Independent Living (SIL) in Gosford, NSW. You will support daily routines, personal care, community participation, and documentation under person-centred care plans.

Willingness to obtain required checks (Working With Children, Police, NDIS) is expected; drivers license advantageous.

Qualifications

• Experience in supporting people with disability.
• Knowledge of person-centred care.
• Ability to work as part of a team.

Responsibilities

• Supporting participants with daily routines, including personal care and meals.
• Assisting participants to build independence and life skills.
• Supporting community participation and activities aligned with participant goals.
• Completing accurate shift notes, progress reports and incident reports.
• Communicating effectively with participants, families and team members.
