Job summary

Unique Support Services Pty Ltd is hiring a full-time Support Worker in Western Australia to assess client needs and develop person-centred support plans. You will coordinate access to community services, provide life-skills coaching, and advocate for inclusion.

Qualifications

• Minimum 1 year paid full-time experience in community services or related fields.
• Relevant qualification equivalent to AQF Level 5 or higher.
• Available for full-time hours 9am–5pm, Monday to Friday.
• English language competence is required.
• NDIS Worker Screening Check or willingness to obtain.
• Working with Children Check or willingness to obtain.
• Current First Aid & CPR or willingness to obtain.
• Reliable vehicle and driver's licence required.

Responsibilities

• Assess clients’ needs and develop individualised support, education and training plans.
• Provide guidance, life skills coaching and emotional support to clients and families.
• Coordinate access to community, welfare, housing, employment and other services.
• Monitor, document and report client progress.
• Advocate for client inclusion, independence and community participation.
• Liaise with families, community organisations and government agencies.
• Prepare reports, funding submissions and recommendations to support client outcomes.
