• Strong communication and reporting skills.

Responsibilities

• Conduct comprehensive client assessments to identify needs, barriers and goals.
• Develop, implement and review personalised support plans including education, vocational and life-skills programs.
• Coordinate with health, housing, employment and training services to build community resources.
• Advocate for clients and monitor progress to support independence.
