Qualifications

• Diploma or relevant qualification in Community Services, or Aged Care.
• Minimum 1 year full-time relevant experience in community or residential care.
• Excellent communication and interpersonal skills, capable of building rapport with clients from diverse backgrounds.
• Ability to assess complex needs, develop effective support plans, and monitor progress objectively.
• Knowledge of relevant legislation, ethical practices, and confidentiality standards in social and community services.

Responsabilités

• Conduct comprehensive assessments of clients’ needs through interviews and observations, identifying challenges related to social, emotional, health, housing, employment, and training issues.
• Develop and implement tailored care plans, educational programs, and support strategies to promote clients’ growth and independence.
• Monitor clients’ progress regularly, maintaining detailed records and providing reports to relevant stakeholders; adjust care plans as necessary to ensure optimal outcomes.
• Facilitate access to community resources by referring clients to specialized agencies and services, including health, welfare, housing, employment, and training providers.
• Collaborate with community groups, welfare organizations, government departments, and private sector partners to promote awareness of available community resources and advocate for client needs.
• Support families by providing education, guidance, and assistance in navigating social services, and deliver care and supervision to children and disabled individuals in various residential settings.
• Supervise offenders on probation or parole, ensuring compliance with legal and program requirements while supporting their reintegration into the community.
• Assist young people facing social, emotional, or financial difficulties by offering tailored support, crisis intervention, and development of coping strategies.
• Prepare detailed submissions for funding applications, compile reports for government agencies, and contribute to the organization’s strategic planning and resource allocation.
