The Baker Heart and Diabetes Institute in Melbourne is seeking a Credentialled Diabetes Nurse Educator to lead diabetes education for individuals and groups across type 1, type 2 and gestational diabetes. You will collaborate with an allied health team and contribute to ongoing professional training.

You will deliver education sessions, keep precise records, and support continuous quality improvement while staying current with diabetes technologies and guidelines.

Qualifications

• Accredited Diabetes Nurse Educator with clinical education experience.
• Division 1 RN with AHPRA registration and diabetes education qualification.
• Experience with insulin pump therapy and CGM preferred.
• Experience or willingness to run group education for clients or health professionals.
• Willingness to learn diabetes technologies and related databases.
• Strong planning and organizational skills with attention to detail.
• Team player with ability to work in an interdisciplinary team.

As a Diabetes Nurse Educator, you will be responsible for conducting client individual and group education services and presenting at relevant health professional training programs. You will become a valued member of the innovative Allied Health and Education Services team at the Baker Heart and Diabetes Institute. The position holder will be primarily based within clinical services at the Prahran site, however travel may be required within the Melbourne Metropolitan area to provide consultations and within Australia to attend health professional training and conferences.

Key responsibilities

• Conduct individual and group clinical sessions and programs as rostered by the Manager, Allied Health in accordance with relevant policies, procedures and lesson plans.
• Demonstrate effective communication skills with clients and the ability to develop and maintain rapport including individual consultations and group education sessions.
• Provide client-centred education empowering clients to self-manage their condition.
• Ensure own knowledge and skills in diabetes education are of a high standard and in accordance with current clinical guidelines.
• Demonstrate commitment to the principles of quality improvement and participate in quality activities.
• Maintain accurate and timely records and data entry for internal and external reporting, including patient records, letters, appointments, services and weekly statistics.
• Supervise students and ensure practices are in accordance to professional standards and competencies.

About you

• Accreditation as a Credentialled Diabetes Nurse Educator with experience providing clinical support and education for people with type 1 diabetes, type 2 diabetes and gestational diabetes preferred.
• Division 1 Registered Nurse with current registration with AHPRA and a post graduate certificate in Diabetes Education.
• Experience in insulin pump therapy and CGM technology is preferred.
• Experience or willingness to deliver group education programs or health professional training face to face or by online technology.
• Experience or willingness to learn diabetes technologies including insulin pumps and continuous glucose monitoring and related databases.
• Excellent planning and organizational skills, including the ability to manage a range of tasks with conflicting priorities without compromising close attention to detail and accuracy.
• Demonstrated ability to be a proactive team member and contribute to team development and work effectively in an interdisciplinary team.
• Demonstrated ability and willingness to take initiative and to improve and enhance existing systems and procedures.
• A high level of interpersonal skills, with the ability to liaise effectively with a wide range of people at a variety of levels internal and external to the Institute.
