Permanent Part time 32 hours per fortnight | Community Health | Relocation Support Available

An exciting opportunity exists for an experienced and motivated Diabetes Educator to join a dedicated and supportive Community Health team in regional Victoria, approximately three hours' drive from Melbourne.

This position plays a pivotal role in providing person‑centred diabetes education and clinical care across acute, community and aged care settings. You'll work both autonomously and as part of a multidisciplinary team to enhance access to care, promote self‑management, and improve quality of life for consumers across the region.

Key Responsibilities
  • Deliver high-quality, evidence-based diabetes education and clinical support to individuals and groups across acute, community and aged care settings.
  • Collaborate closely with General Practitioners, nurses, allied health, and community services to ensure coordinated, person‑centred care.
  • Conduct comprehensive assessments, develop care plans, and support consumer self‑management strategies.
  • Provide education on insulin administration, glucose monitoring, sick day management and lifestyle modification.
  • Utilise telehealth and digital tools to reach clients across the catchment area.
  • Participate in health promotion, risk management, and quality improvement initiatives.
  • Maintain accurate documentation and comply with all relevant legislation, policies, and professional standards.
Qualifications
  • Current AHPRA registration as a Registered Nurse (Division 1).
  • Graduate Certificate in Diabetes Education (or working towards completion).
  • Eligible for membership with the Australian Diabetes Educators Association (ADEA).
  • Demonstrated experience in diabetes education within a community or primary health setting.
  • Proven ability to work both independently and collaboratively within a multidisciplinary team.
  • Excellent communication, interpersonal and organisational skills.
  • Proficiency in Microsoft Office and clinical documentation systems.
  • Current Victorian Driver's Licence, Police Check, and Working with Children Check (or willingness to obtain).
  • Experience working in rural or regional health settings.
  • Knowledge of culturally safe practice and chronic disease prevention strategies.
