Rich River Health Group in Echuca, VIC is seeking a passionate Diabetes Educator to join our team as a Full-Time role. You will educate patients living with diabetes, develop management plans, and support ongoing care in collaboration with GPs, nurses and allied health staff.

You will provide education to patients and families, assist with lifestyle modification strategies, and maintain thorough documentation as part of delivering high-quality, patient-centred care.

Qualificações

• Diabetes Educator accreditation and specialist registration.
• Excellent communication and interpersonal skills.
• Ability to work both independently and with a multidisciplinary team.

Responsabilidades

• Provide diabetes education and support to patients and families.
• Develop and implement individualised diabetes management plans.
• Assist patients with lifestyle modification strategies and self-management.
• Coordinate patient care with GPs, nurses and allied health team.
• Maintain accurate clinical documentation and records.
• Support preventive health initiatives within the clinic.
