You will collaborate with a multidisciplinary team to educate patients and families, support self-management, coordinate care, and contribute to a high‑quality diabetes education program.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Excellent verbal and written communication skills.
• Demonstrated ability to work effectively as part of a multi-disciplinary team.

Responsibilities

• Provide diabetes education and support to patients and their families.
• Support patients to build confidence in diabetes self-management.
• Collaborate with the multidisciplinary team to coordinate and optimise patient care.
• Contribute to the ongoing development and delivery of a high-quality diabetes education service.
• Develop and provide a broad range of clinical services to people with diabetes.
