Job summary

South Pacific Private on the Northern Beaches of Sydney is seeking a Day Program Co-ordinator and Continuing Care Case Manager to lead program administration and support clients through the first 90 days of recovery. Working with a multidisciplinary team, the role coordinates admissions, scheduling, and care planning, ensuring high-quality, client-centred care from inpatient admission onward.

Qualifications

• Bachelor’s degree in psychology, counselling, psychotherapy, or related discipline.
• Experience in addiction, mental health, trauma-informed care, or behavioural health settings.
• Strong case management and client engagement skills.
• Excellent interpersonal, written, and verbal communication.
• Ability to work collaboratively within a multidisciplinary team.
• Strong organisational, planning and time management skills.
• Intermediate computer skills and accurate clinical documentation.

Responsibilities

• Coordinate Day Program administration and staffing with multidisciplinary teams.
• Manage client admissions, scheduling, group allocations and outcomes.
• Liaise between clinical and administrative teams for smooth delivery.
• Roster staff, calendar management, workshop scheduling and day program operations.
• Monitor progress, discharge planning and quality improvement activities.
• Provide clinical and operational support for high-focus clients.
