Responsabilités

• Provide holistic and collaborative case management support to clients with complex needs.
• Undertake assessments and develop individual support plans that promote rehabilitation.
• Deliver evidence-informed interventions and assess/manage risk.
• Prepare reports on client progress and present information in a court setting.
• Build strong partnerships with courts, community services and referral agencies.
• Maintain accurate records, documentation and confidentiality standards.
• Deliver culturally responsive, inclusive and person-centred support.
