Job summary

Centacare Brisbane is seeking an experienced Service Delivery Coordinator to join our Family and Relationship Services team at our Fortitude Valley office. You will coordinate day-to-day operations, support staff, and ensure service delivery meets funding and regulatory requirements.

You will support intake, risk assessment, safety planning, and management of complex cases while building strong relationships with partner agencies.

Qualifications

  • Minimum 5 years’ frontline practitioner experience, plus 2 years in a senior role.
  • Tertiary qualification in Social Work, Psychology, Social Science or Behavioural Science; APS/AASW/PACFA/ACA eligible.
  • Current driver’s licence and willingness to obtain a Working with Children Check.
  • Ability to work 8:30–17:00, Mon–Fri, with occasional 12:00–20:00 shifts (Tue/Thu).
  • Current working rights in Australia and ability to obtain a National Police Check.

Responsibilities

  • Coordinate day-to-day service operations and ensure effective delivery.
  • Lead intake, risk assessment, safety planning, and complex case management.
  • Provide supervision, leadership, mentoring, and practice support to practitioners.
  • Support intake, assessments, and direct client work as required.
  • Oversee records, data collection, reporting, and compliance with funding.
  • Develop referrals and partnerships with internal and external stakeholders.

The Role

We are currently seeking an experienced Service Delivery Coordinator to join our Centacare Family and Relationship Services team at our Fortitude Valley office.

In this role, you will coordinate the day‑to‑day operations of the service, support staff, and ensure service delivery meets funding, legal and organisational requirements.

You will support intake, risk assessment, safety planning, and the management of complex and high‑risk cases while building strong relationships with partner agencies.

The role also includes providing guidance, supervision, and practice support to practitioners and may involve some direct client work when required.

Our clients include individuals, couples, children and families requiring family and relationship support as well as those experiencing domestic and family violence (DFV).

Key Duties
  • Coordinate the day‑to‑day operations of the service and ensure effective service delivery.
  • Lead and support intake, risk assessment, safety planning, and complex case management.
  • Provide supervision, leadership, mentoring, and practice support to practitioners.
  • Support intake, assessments, and direct client work as required.
  • Oversee administration, case records, data collection, and reporting requirements.
  • Coordinate workforce requirements, stakeholder engagement with funded programs, referrals, rosters, and community partnerships.
  • Develop and maintain effective referral pathways and strong relationships with internal and external stakeholders.
  • Ensure service delivery aligns with relevant legislation, funding requirements, organisational policies, and professional standards.
Skills and Experience
  • Experience leading or coordinating services within the counselling, community, or human services sectors.
  • Strong understanding of domestic and family violence (DFV) practice, including risk assessment, safety planning, and case management.
  • Proven experience supervising, mentoring, and supporting practitioners.
  • Strong organisational, communication, and prioritisation skills, with the ability to work effectively in a busy service environment.
  • Experience working with funding requirements, contracts, reporting, data collection, and compliance.
  • Strong stakeholder engagement skills, including referral pathway development, documentation, and client management systems.
  • Ability to manage competing priorities and respond appropriately to complex and high‑risk situations.
Compliance Requirements
  • A minimum of 5 years' experience as a frontline practitioner and 2 years' experience at a senior level.
  • Tertiary qualifications in Social Work, Psychology, Social Science, or Behavioural Science from a recognised institution, including eligibility for membership of APS, AASW, PACFA, or ACA.
  • Ability to work 8:30 am - 5:00 pm, Monday to Friday, with the flexibility to work from 12:00 pm - 8:00 pm (only Tuesdays or Thursdays) when required.
  • Current driver's licence.
  • Willingness to obtain a Working with Children Check.
  • Current working rights in Australia.
  • Ability to obtain a National Police Check issued within the previous three months.
