Pathway Talent is seeking compassionate Support Workers to assist clients across Newcastle, helping with daily living, personal care and community participation. You will build trust, respect independence, and work within individual support plans to ensure high-quality, person-centred care.

Qualifications

• Minimum Cert III in Individual Support or similiar.
• Experience supporting people living with disability and/or mental health challenges.
• Strong communication skills and the ability to build positive relationships.
• The ability to work independently and as part of a team.
• A reliable approach to maintaining professional boundaries and delivering consistent support.
• A current driver’s licence.
• A relevant qualification, such as a Certificate III in Individual Support or Certificate IV in Disability, or equivalent experience.

Responsabilités

• Supporting customers with daily living activities, including personal care, meal preparation and household tasks.
• Encouraging community participation, social connections and access to appointments.
• Helping customers build skills and work towards their individual goals.
• Following individual support plans and behaviour support strategies where required.
• Maintaining accurate progress notes and reporting changes in customer wellbeing.
• Working collaboratively with customers, families and the broader support team.
