Job summary

Pathways to Better Living Pty Ltd – Banora Point NSW is seeking a dedicated Disability Support Worker to provide personalised in-home and community-based support, helping participants to maintain independence and a meaningful quality of life.

You will follow individual plans, assist with daily living, personal care, meal prep, and community activities while ensuring safety and dignity for participants. Training, supervision and supportive management are provided.

Qualifications

• Previous experience working with people with disability, preferably in SIL or residential settings.
• A strong understanding of person-centred support.
• Excellent communication and interpersonal skills.
• The ability to work independently while also being part of a team.
• A calm and responsible approach, particularly when situations are challenging.

Responsibilities

• Providing person-centred daily living support
• Supporting participants with personal care and household tasks
• Assisting with meal preparation and maintaining a safe, clean home environment
• Supporting participants to access the community and participate in activities
• Following individual support plans, routines and behaviour support strategies
• Supporting medication routines where appropriately trained
• Encouraging independence, choice and control
• Maintaining accurate progress notes and required documentation
• Working collaboratively with participants, families and the wider support team
• Responding appropriately to changes in participant needs or circumstances
