Job summary

Discover Nursing Australia is seeking an experienced Disability Support Worker for in-home and community participation in Hamilton, VIC. You will provide 1:1 support after dialysis and during daytime appointments, shopping, and activities, adapting to complex physical and psychosocial needs.

The role emphasises reliability, calm communication and strong organisational skills, with guidance from our nursing and coordination team. Must be comfortable around dogs and have relevant qualifications.

Qualifications

• Certificate III in Individual Support or equivalent.
• NDIS Worker Screening Check current.
• National Police Check current.
• Current First Aid and CPR certification.
• Completion of the NDIS Worker Orientation Module – Quality, Safety and You.
• Australian working rights and ABN (or willingness to obtain).
• Current driver’s licence and a reliably insured vehicle suitable for participant transport.

Responsibilities

• Provide reliable collection and transport home following dialysis.
• Assist with dinner preparation in line with documented dietary guidance.
• Give respectful prompts for showering, medication routines and daily tasks as per the support plan.
• Maintain an up-to-date calendar of appointments and commitments.
• Support shopping, errands and attendance at medical appointments.
• Encourage participation in community activities aligned with the participant’s interests.
• Use clear communication and allow time for the participant to respond.
• Observe and report changes in wellbeing per monitoring instructions.
• Maintain progress notes and communicate concerns to the team.
• Collaborate with the participant, plan nominee, and nursing/coordination team.
