Job description

Disability Support Worker – In-Home Support & Community Participation

Hamilton, VIC 3300 | Approximately 12 Hours per Week | Ongoing 1:1 Support

Provide consistent, practical support to a gentleman establishing his NDIS supports, with a focus on daily routines, independence and community connection.

Discover Nursing Australia is seeking experienced, reliable and compassionate Disability Support Workers to provide support in Hamilton.

This role combines evening assistance following dialysis with daytime support for appointments, shopping and community activities. It would suit someone with a calm manner, strong organisational skills and experience supporting people with complex physical and psychosocial needs.

About Discover Nursing Australia

Discover Nursing Australia is a nurse-led disability support provider committed to delivering person-centred support that respects each participant’s dignity, preferences and choices.

We engage Disability Support Workers as independent contractors, with guidance from our Registered Nurse and coordination team.

About the Participant

The participant lives in a shared home and requires regular prompting and practical assistance to maintain daily routines, manage appointments and participate in activities of his choosing.

He attends haemodialysis three times per week and lives with complex health conditions, chronic pain and psychosocial support needs. Consistency, patience and clear, respectful communication are essential.

His border collie is an important source of emotional support and is expected to return to the home. Applicants must be comfortable working around dogs.

The participant has expressed a preference for a mature female support worker, where possible. Compatibility and the ability to establish a trusting professional relationship will be important considerations.

Key Responsibilities:

  • Provide reliable collection and transport home following dialysis.

  • Assist with dinner preparation in accordance with documented dietary guidance.

  • Provide respectful prompts for showering, medication routines and other daily tasks, as outlined in the support plan.

  • Help maintain an up-to-date calendar of appointments and commitments.

  • Support shopping, errands and attendance at medical appointments.

  • Encourage participation in community activities that reflect the participant’s interests.

  • Use clear communication and allow sufficient time for the participant to respond and complete tasks.

  • Observe and report changes in wellbeing, following documented monitoring and escalation instructions.

  • Maintain accurate progress notes and communicate relevant concerns promptly.

  • Work collaboratively with the participant, his plan nominee and the nursing and coordination team.

Participant-specific guidance will address relevant health concerns, including abdominal swelling and changes in breathing or appetite. Workers must follow the documented escalation plan and remain within their training and role.

About You

We are seeking workers who demonstrate:

  • Experience supporting people with complex physical and psychosocial needs.

  • A calm, patient and respectful approach.

  • Confidence providing prompting without taking over tasks the participant can complete.

  • Strong organisational skills and attention to changes in presentation.

  • Reliability for evening shifts and agreed daytime support.

  • Sound judgement, professional boundaries and effective communication.

  • Respect for privacy and the participant’s shared living environment.

  • Comfort working in a home with a dog.

Essential Requirements:

  • Certificate III in Individual Support or an equivalent relevant qualification.

  • Current NDIS Worker Screening Check.

  • Current National Police Check.

  • Current First Aid and CPR certification.

  • Completion of the NDIS Worker Orientation Module – Quality, Safety and You.

  • Australian working rights.

  • Active ABN, or willingness to obtain one before commencing.

  • Current driver’s licence.

  • A reliable, comprehensively insured vehicle suitable for participant transport.

What We Offer:

  • Ongoing 1:1 participant support with a defined weekly schedule.

  • Competitive contractor rates.

  • Applicable travel and participant transport reimbursement.

  • Professional Indemnity and Public Liability Insurance provided.

  • Guidance from our Registered Nurse and coordination team.

  • Participant-specific support and professional development opportunities.
