Job summary

Lutheran Aged Care invites applications for a Community Home Care Worker to deliver direct care in Culcairn, Holbrook and surrounding areas. You will follow client care plans, support independence, and liaise with the Community Services Coordinator to ensure high-quality home care services.

The role requires a Certificate III in Aged Care and at least 6 months in health, residential or community aged care, with a current police check preferred.

Qualifications

• Certificate III in Aged Care or equivalent qualification required.
• Minimum 6 months experience in health, residential or community aged care.
• Police check (current) preferred or required by policy.

Responsibilities

• Provide direct care and support to clients as per care plans.
• Ensure client safety and privacy during visits and assist with daily living tasks.
• Maintain accurate documentation and report changes to the Coordinator.
• Participate in ongoing training and adhere to enterprise agreement terms.
