Job summary

Silverchain seeks a reliable Home Care Worker to support older clients in their own homes in Blacktown and surrounding areas. You will provide personal care, domestic tasks, and companionship with a focus on independence and dignity.

Applicants should hold a Cert III in Individual Support (Ageing) or higher, plus First Aid/CPR. A valid Australian driver’s licence and a vehicle are preferred. Training opportunities and robust support are offered.

We are looking for a Cert III qualified person with at least 6 months experience in a similar role, to join the Silverchain team in Blacktown and surrounding areas.

In addition, you will need:

• A valid Australian Driver's Licence & your own insured, roadworthy vehicle
• Certificate III or above in Aged Care or Individual Support or Disability
• Experience of 6 months or more in a similar role, with experience in personal care, manual handling, hoist training and medications.

The role

In the role of Care Worker you will be providing our aged care clients with home help services such as:

• General housekeeping and making beds
• Cleaning, mopping and vacuuming floors
• Washing, folding and ironing laundry
• Meal preparation and grocery shopping.
• You will also be providing our aged care clients with personal care services such as:
• Getting clients out of bed, showered and dressed for the day
• Assisting clients to take their medications
• Assisting with mobility and therapy goals
• Providing complex care for customers with high care needs, such as dementia
• Providing social and emotional support
• Providing the client with transport to lifestyle activities or appointments
• Helping the family carer to have a break or an opportunity to do other things.

Position Summary: The Home Care Worker provides person‑centered support to older people living in their own homes, assisting them to maintain independence, dignity, and quality of life. The role involves delivering practical, emotional, and personal care in line with individual care plans, organisational policies, and aged care standards.

Key Responsibilities:

Personal Care:

• Assist clients with activities of daily living, including showering, grooming, toileting, dressing, and mobility.
• Support safe transfers and use of mobility aids as required.
• Promote comfort, dignity, and respect at all times.

Domestic Support:

• Assist with light household duties such as:
• Cleaning and tidying
• Laundry and ironing
• Meal preparation in accordance with dietary requirements
• Support clients to maintain a safe and hygienic home environment

Social Support & Companionship:

• Provide companionship and emotional support
• Encourage social engagement and participation in community activities
• Accompany clients to appointments or outings when required
• Observe and report changes in client health, behaviour, or wellbeing
• Prompt or assist with medication according to care plans (where authorised)
• Follow infection control and work health and safety procedures

Documentation & Communication:

• Complete care notes accurately and in a timely manner
• Communicate effectively with supervisors, care coordinators, clients, and families
• Adhere to confidentiality and privacy requirements
