Job summary

integratedliving Australia is seeking 2 permanent part-time Support Workers to provide person-centred home care in Ballarat and surrounding areas. You will travel between clients, delivering personal care, domestic support, and community access with a focus on maintaining independence.

Ideal candidates hold a Certificate III in Individual Support or equivalent, have a valid driver’s licence, and are comfortable using mobile documentation tools.

Qualifications

• Certificate III in Individual Support / Aged Care (or equivalent).
• Current driver’s licence and reliable car with comprehensive insurance.
• First Aid/CPR and Working With Children Check (WWCC) (or willingness to obtain).
• National Police Check or NDIS Worker Check (or willingness to obtain).
• Comfortable using smartphones or tablets for documentation.
• A caring, reliable, and respectful approach to supporting clients.

Responsibilities

• Provide personal care and medication assistance based on individual care plans.
• Assist with domestic support including cleaning, vacuuming, mopping, and general household tasks.
• Support community access, such as grocery shopping, appointments, and social outings.
• Help clients maintain independence and wellbeing at home.
• Record visit notes using our simple mobile app.
