Job summary

Mercy Health is seeking a Care Partner to support clients in the Young NSW region. This full-time role focuses on managing and supporting Home Care clients while identifying opportunities to grow our services in the community.

The position requires dedication to high-quality, person-centred care delivered with empathy. Ideal candidates have at least 2 years in aged care case management or disability services, and hold a relevant qualification in community services or case management.

Qualifications

• In-depth knowledge of the current Support At Home model of care.
• Minimum 2 years' experience in aged care case management or disability services.
• A relevant qualification in community services or case management.
• Excellent time and client budget management, organisational and communication skills.
• Compassionate, flexible and positive approach to patient care and management.
• Ability to nurture respectful relationships and work in a collaborative team.
• Ability to work autonomously and in a dynamic team environment.
• Evidence of or in process of obtaining: current Police Record Check.
