Job summary

Mercy Health Home Care Young NSW is seeking a Care Partner to support clients and expand our Home Care services in the Young region. You will deliver compassionate, person-centred care and work with a collaborative team to enhance community support.

You’ll bring at least 2 years in aged care case management or disability services, with qualifications in community services or case management. Police check required and the role is full-time.

Qualifications

• Minimum 2 years’ experience in aged care case management or services coordination.
• Qualification in community services or case management.
• Strong time and budget management, organisational and communication skills.
• Ability to work autonomously and in a team.
• Evidence of Police Check.

• Support at home clients in the Young region.
• Identify opportunities to expand Home Care services.
