Job summary

Access Foundation WA is seeking a compassionate Disabled Carer to support people with disabilities in their home and local community. You will help clients maintain independence, dignity, and quality of life, carrying out personal care, mobility support, meal prep, and medication assistance as part of a dedicated care team.

The role requires strong communication skills, a Certificate III or higher in related fields, and a willingness to work with individuals with diverse needs.

Qualifications

• Certificate III or higher qualification in Individual Support, Disability, Aged Care, or related field.
• Experience working with people with disabilities preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Current First Aid and CPR Certificate.
• National Police Clearance and NDIS Worker Screening Check.
• Current driver's licence and reliable vehicle preferred.

Responsibilities

• Provide personal care assistance including showering, dressing, grooming, and toileting.
• Assist clients with mobility and transfers using appropriate manual handling techniques and equipment.
• Support clients with meal preparation, feeding, and household tasks.
• Administer or assist with medications as directed in care plans.
• Accompany clients to medical appointments, community activities, and social outings.
• Monitor and report changes in clients' health, behaviour, and wellbeing.
• Maintain accurate client records and progress notes.
• Provide emotional support and promote client independence and choice.
• Follow individual support plans, workplace policies, and safety procedures.
