Job summary

Claro Disability Services is seeking a Community Support Worker to assist clients in Traralgon, Moe, and Churchill. You will support mobility, daily tasks, and community access, while promoting client goals with empathy and respect.

Ideal candidates are caring, reliable, and adaptable, with experience in complex care and strong communication. A current Australian driver's license and a reliable vehicle are preferred. Shifts are arranged to match client needs, across multiple towns.

Responsibilities

• Chat with the client (and family/advocates) to understand goals and routines.
• Help with mobility, transfers, positioning, daily tasks, and devices as per the plan.
• Monitor medical plans, liaise with the care team about changes.
• Facilitate social time and community access aligned with client interests.
• Support autonomy and advocacy while respecting consent and independence.
• Respect cultural backgrounds, beliefs, and ways of being.
