Responsibilities

• Provide care and assistance to participants and families.
• Assess community needs and resources for health, welfare, housing, employment, training and other services.
• Arrange activities to enhance physical, social and emotional well-being.
• Monitor and report client progress.
• Refer clients to agencies for additional help.
