Job summary

Achieve Australia is seeking an Assistant in Nursing to provide person-centered quality support to people with a disability. You will help clients set goals and live the life they choose. You will work in a group home, promoting independence and building relationships through respectful, safe care.

Role includes personal care, transport, documentation, and advocacy, with paid induction and ongoing training. NSW driver’s licence required; remote work is not offered.

Qualifications

• Relevant qualifications (minimum Cert III in Disability or Individual Support).
• Strong interpersonal, role modelling, and customer service skills.
• Problem-solving abilities with an ability to adapt to changing environments.
• Understanding of National Disability Insurance Scheme (NDIS) and Code of Conduct and Workplace Health and Safety.
• Awareness of human rights and challenges facing individuals with disabilities.
• Ability to transport clients in Achieve Australia vehicles (automatic gearbox); NSW driver’s licence required or risk assessment if not held.
• Experience in working with and supporting adults and/or children with complex disability.

Responsibilities

• Provide person-centered quality support services to people with a disability.
• Support clients to set and reach goals that matter to them and help them live the life they choose.
• Work in a group home setting and support clients to build independence and relationships.
• Provide an enjoyable, respectful, and safe environment for all people we support.
• Provide personal/physical care and support to clients including grooming, toileting, meal preparation and assistance, home support, transport and medication administration.
• Support the use of physical and communication aids and devices and other personal equipment where required.
• Provide support enabling clients to participate in hobbies, interests, and community activities.
• Assist clients to access recreational services and transport services, including public transport and Achieve Australia vehicles.
• Advocate for and assist clients, their families, and significant people to engage with the community.
• Complete client documentation including case notes, updating the client management system, and medical documentation.
