Job summary

Vision and Care seeks a dedicated Community Worker to join our team in Adelaide, SA. You will assess client needs, coordinate access to disability and community services, and advocate for independent, inclusive participation in daily life.

Key duties include referrals, case management, documentation, and collaboration with local organisations and professionals. A person-centred, culturally sensitive approach and the ability to work both independently and in a team are essential.

Qualifications

• Relevant qualifications in Community Services, Social Services, Welfare, or a related field.
• Relevant experience in community services, disability services, welfare or a similar environment.
• Strong communication and interpersonal skills.
• Ability to assess client needs and coordinate appropriate services and referrals.
• Ability to communicate effectively with clients, families, community organisations and other service providers.
• Good organisational, documentation and case management skills.
• A person-centred and culturally sensitive approach.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Assess clients' needs and identify suitable community, health, welfare, housing and social support services.
• Help clients set goals and develop strategies to improve independence and participation.
• Provide information, referrals and guidance to services and organisations.
• Coordinate with community organisations, welfare agencies and health professionals.
• Assist clients to access programs, education, employment and recreational opportunities.
• Build relationships with local organisations and service providers.
• Coordinate community-based activities promoting inclusion and participation.
• Monitor progress and adapt support plans to changing needs.
• Advocate for clients to overcome access barriers to resources.
• Maintain accurate client records and reports as required by policy.
• Contribute to development and improvement of services.
• Promote awareness of available resources.
