Job summary

Wellcare Australia Pty. seeks motivated Community Workers to provide holistic care within residential settings, collaborating with a team to support participants in their recovery journey.

Key responsibilities include assessing needs, developing programs, interviewing clients, and coordinating with agencies to access additional help. A driver’s license and valid checks are required for frontline fieldwork.

Qualifications

• High empathy and motivation to support participants' recovery.
• Experience in residential care or community settings is desirable.
• Valid Working with Vulnerable People clearance and background checks.
• Driver's license and own insured vehicle.

Responsibilities

• Assess clients' needs and plan, develop and implement educational, training and support programs.
• Interview clients and assess the nature and extent of difficulties.
• Monitor and report on client progress.
• Refer clients to agencies for additional help.
• Assess community needs and resources for health, welfare, housing, employment, training and other facilities.
• Liaise with community groups and government bodies to promote resources and services.
• Support families and provide education for disabled persons in adult service units, group housing and government institutions.
• Assist people to solve social, emotional and financial problems.
• Prepare submissions for funding and resources and reports to government bodies.

The core mission of Wellcare Australia is to promote and support a recovery based on hope, and an ambition for flourishing mental health, for our participants. We offer shared housing and assistance for individuals with ongoing mental health needs and are a registered NDIS provider.

Role Overview

We seek motivated and qualified Community Workers. The position involves providing holistic care within residential settings, working collaboratively with a team to support participants in their recovery journey.

Key Responsibilities

Assessing clients' needs and planning, developing and implementing educational, training and support programs

Interviewing clients and assessing the nature and extent of difficulties

Monitoring and reporting on the progress of clients

Referring clients to agencies that can provide additional help

Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services

Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services

Supporting families and providing education and disabled persons in adult service units, group housing and government institutions

Assisting people to solve social, emotional and financial problems

Preparing submissions for funding and resources, and reports to government bodies and other agencies

Requirements

- High degree of empathy and motivation to contribute to our participant's recovery journey.

- A sincere commitment to supporting individuals facing mental health challenges.

- Experience in residential care or community settings is highly desirable.

- Valid Working with Vulnerable People clearance, National Criminal History Check, First Aid and CPR certification, Covid-19 Vaccination, NDIS Orientation Module, and COVID-19 Infection Control Module.

- Valid full Australian driver’s license and own comprehensively insured vehicle.
