Job summary

Eyre and Far North Local Health Network is seeking a part-time Community Support Worker in Kimba to deliver in-home and community-based services under the Consumer Directed Care model.

You will assist with personal care, meal prep, cleaning, transport and social support, helping clients stay independent. You will work with consumers, families and health professionals to plan and review care, travel to clients as needed and contribute to a supportive, patient-centred team.

In this rewarding role, you will support individuals who are aged, frail, or disabled by providing essential services such as personal care, meal preparation, cleaning, transport, and social support. Through our Consumer Directed Care model, you will work directly with consumers to help them maintain independence and make choices about their care.
